Jeffrey Formulak, 61, of Charles Town, WV passed away Monday, September 18, 2023 at his residence.

Born November 6, 1961 in Olney, MD, he was the son of the late Larry and Connie Formulak.

Jeff was a loving father and an avid Pitt Panther College Football fan. He had a passion for music and riding his Harley.

He is survived by his wife, Donna J. Kable- Formulak, at home; two sons, Jared Formulak and wife, Anna  and Jonathan Formulak and special bonus daughter, Cait, one sister, Jennifer Roeder and companion, Randy, one brother, Colin Formulak and wife, Christine,  two nieces and two nephews.

In addition to his parents, he is preceded in death by his  maternal and paternal grandparents.

Services will be private and at a later date.
